Your Right to a Good Faith Estimate

You have the right to receive a written estimate of the expected cost of your care. This generally applies if you do not have health insurance or choose not to use it for the services.

When you receive an estimate

• If you schedule care 3 to 9 business days ahead, the estimate is due within 1 business day after scheduling.
• If you schedule care at least 10 business days ahead, the estimate is due within 3 business days after scheduling.
• You may request an estimate before making an appointment. It is due within 3 business days after your request.

Your estimate should describe the services and expected charges. For recurring therapy, it may include the anticipated frequency and period of care. Ask for clarification if your treatment plan or expected costs change. Keep a copy to compare with later bills.

If a bill is higher than your estimate

If a provider or facility bills you at least $400 more than its Good Faith Estimate, you may qualify for the federal patient and provider dispute resolution process. You generally must start that process within 120 calendar days of the date on the initial bill.

An estimate is not a contract requiring you to receive services. This webpage explains your rights; it does not replace an estimate prepared specifically for you.
